Seamless Transition Web Site - provides benefits information and assistance for returning Active Duty, National Guard and Reserve service members of Operations Enduring Freedom and Iraqi Freedom.

Seamless Transition Contacts - this link provides a list of contacts within VISN 15 that will help coordinate health care and benefits for seriously injured or ill service members returning from theaters of combat operations. 

Polytrauma System of Care - Polytrauma care is for veterans and returning service members with injuries to more than one physical region or organ system, one of which may be life threatening, and which results in physical, cognitive, psychological, or psychosocial impairments and functional disability. Some examples of Polytrauma are Traumatic Brain Injury, Hearing Loss, Amputations, Fracturs, Burns and Visual Impairement.
