Welcome to the VA Heartland Network (VISN 15) Formulary website.

Pharmacy Benefits Management Services 

Veterans who qualify for non-VA provider care under the Veterans Access Choice and Accountability Act (VACAA), Patient-Centered Community Care (PC3), or Non-VA Community Care (NVCC) program need to have their medications filled at a VA Pharmacy unless the Veteran chooses to fill his/her prescriptions at non-VA pharmacies, at their own expense. To expedite the ability to fill medications at a VA pharmacy, non-VA prescribers should complete and fax the below Non-VA Prescriber Registration Form to the facility where the medications is to be dispensed.

It is the policy of the VA to use only those medications that have been approved by the National VA-Pharmacy Benefits (PBM) Services. Furthermore, VA policies require the use of VA National Formulary medications when clinically appropriate. If necessary, non-VA prescribers may request Non-formulary medications by completing and faxing the following form:
